Chronic Care Initiative Programs
Community Health Workers (CHWs) provide health navigation and support to Durham residents living with chronic health conditions or are at risk of developing Type II Diabetes.
What is a Community Health Worker?
A frontline public health worker who has both the trust and deep understanding of the community being served. This personal relationship with the community is established through outreach, community education, networking and in many cases "lived experience" that gives them greater knowledge of the resouces needed to support the clients served.
Our community health workers partner with you, your health care providers, and community resources to ensure you understand your health, are connected to the services needed to maintain or change your health, and can navigate systems and services independently so you can enjoy healthier living!
Our Programs
Chronic Care Initiative (CCI)
Community program where Community Health Workers provide health navigation and linkage to resources for Durham County residents living with chronic health conditions.
NC-Minority Diabetes Prevention Program Offered By CCI
A Diabetes Prevention Program aimed at helping to prevent diabetes through a yearlong training. Supported when available by grant funding allowing us to provide engagement incentives that help promote healthier living and attainment of weight loss goals.
Formerly Incarcerated Transition Program (FIT)
Community Health Workers provide health navigation and linkage to resources for Durham residents who have returned to the community from incarceration in the past two years and are living with chronic health conditions.
