Population Health
What is Population Health?
The Association of State and Territorial Health Officials defines population health as the health outcomes of a group of individuals including the distribution of such outcomes within the group with three main components- health outcomes, health determinants and policies.
Population Health at Durham County Department of Public Health
The Population Health Division collaborates with the community and within the Durham County Department of Public Health (DCoDPH) to collect data, analyze and assess programs designed to promote the health and well-being of the entire community through equitable, evidence-based solutions and initiatives. Population Health manages internal and external partnerships to influence public health policy and to ensure a well-trained workforce that provides quality services and care.
Strategies to Improve Durham County Health Outcomes
Community Centered Approaches
- Community-wide health coalition, Partnership for a Healthy Durham
- Community-driven health priorities (Community Health Assessment survey & listening sessions)
- Integrate residents’ stories to contextualize population health statistics
Expanding User-Friendly Public Data
- Develop data dashboards that highlight existing health inequities and progress in addressing community-driven health priorities
- Develop infographics and reports to disseminate data on health inequities and priorities
- Correct the narrative to provide context for existing health inequities
Policy Development
- Work with partners and institutions on policies that maximize population health
- Engage community partners in conducting policy analyses to determine which polices will maximize population health
- Conduct quality improvement efforts to provide the best level of services and programs
Community Engagement:
- Engage the community in an authentic way that acknowledges their expertise
- Center community voices and lived experience in the Community Health Assessment process, Community Health Improvement Plans and decision-making
Health Department Accreditation
- Meet the requirements of the Public Health Accreditation Board (PHAB) population health and equity standards to obtain national accreditation
- Maintain standards for state North Carolina Local Health Department Accreditation (NCLHDA) accreditation with honors
