The Chronic Care Initaitive (CCI) program is a partnership between Durham County Department of Public Health, local agencies, and community stakeholders.

 

 

Chronic Care Initiative Program

The purpose of the program is to support Durham residents who are living with diabetes, hypertension, heart disease, COPD, and other chronic health conditions.

Services Provided:

  • Health care navigation by a community health worker (CHW)
  • Oversight of cases by Social Workers that supervises the CHW's
  • Chronic disease self-managment education
  • Home visits and support at local medical appointments when needed
  • Community resource referral assistance for transportation, housing, employment, food and other needs.

Chronic Care Initiative Goals:

  • Provide ease of access to needed health services
  • Enhance self-managment skills around overall daily living
  • Promote healthy lifestyle choices and changes
  • Maintain or improve overall health

Who is Eligible and Should I Submit a Referral

Basic Eligibility:

  • Age 18 or older
  • A resident of Durham County or employee of Durham County
  • Diagnosed with a chronic health condition

Not sure if a referral is appropriate:

  • Are you or someone you know in need of help locating primary health care or applying for services?
  • Do you have trouble navigating various benefits or services you have and need additional support understanding the process, setting up transportation to your appointments or locating other community resources to support your household needs?
  • Do you sometimes need more guidance on how to manage your chronic conditions in the home or support at your local medical appointments?

 

If your response is yes to any of the questions above, then a referral to the Chronic Care Initiative program is a great option for you.

Bi-lingual Community Health Workers supporting English and Spanish speaking clients:

Interpreters are available to support other lanuages as needed.

  • What chronic condition the client needs support with
  • What immediate services are needed
  • Best phone number or email for contact
  • Address
