How do I make an appointment?

An appointment can be made over the phone by calling 919-560-7601. We also have limited walk-in appointments in some clinics. It is strongly recommended to arrive early for same-day walk-in appointments as they fill up quickly. Lobby 1 opens for registration at 8:00 AM.

What happens during Registration?

Please arrive to Lobby 1 at least 15 minutes prior to your scheduled appointment to ensure you are registered on time. Staff will register you and check you in for your appointment. If you have insurance, please bring your insurance card.

To determine fees for health services if you do not have insurance, bring proof of your income. Accepted proofs of income include pay stubs, copy of paycheck, W2/1099s, or bank statements. After you are registered, checked-in, and your income has been verified, staff will direct you to the appropriate clinic.

How much will clinical services cost?

All accepted third-party insurances are billed where applicable, including Medicaid.

If you are uninsured, the cost of clinical services is based on where you may fall on the sliding fee scale. Your income and household size will determine the cost of services. You must bring proof of income to participate in the sliding fee scale, otherwise you will be charged 100% on the sliding fee scale. Proof of income is requested annually and income will be verified at every visit.

Fees for services and medication are assessed according to the rules and regulations of each program and purchase cost. No one will be refused services due to inability to pay.

How do I pay my bill?

For your convenience, you can pay your bill through My UNC Chart ®.

My UNC Chart ® is also available in Spanish here.

Once you’re logged into your MyChart account, simply click on the Billing tab at the top of the screen to get started. Credit card payments can be made directly and securely through MyChart.

If you do not have a MyChart account, please visit Public Health Registration office in Lobby 1 of the Durham County Human Services Building to sign up and/or pay your bill.

Do you have a sliding fee scale?

Yes, our sliding-fee program allows reduction or "sliding" of the fees for services. Uninsured guests should bring proof of income to participate in the sliding fee scale, otherwise you will be charged 100% of fees for services rendered. Sliding fee scale eligibility is assessed according to the rules and regulations of each program and the recommended program’s Federal Poverty Level Scale (Sliding Fee Scale) will be used to determine fees for all programs. All accepted third-party insurances are billed where applicable.

How do I apply for Medicaid?

Medicaid applications are accepted and processed through the Department of Social Services (DSS), not Public Health (DPH). There are three ways to apply for Medicaid:

414 East Main Street
Durham, NC 2770
