Care Management for High-Risk Pregnancy (CMHRP)

CMHRP is comprised of a group of committed and passionate social workers and nurses also known as Pregnancy Care Managers. Pregnancy Care Managers work with you and your prenatal care provider to ensure that you receive the best care while you are pregnant and after you deliver your baby. 

Care Management for High-Risk Pregnancy (CMHRP) is provided at no cost and participation is voluntary.

 

Your Pregnancy Care Manager may assist you by

  • Helping you get connected to services that you may need such as transportation, food, medical care, or housing
  • Meeting with you after you have been in the hospital or emergency room to see how you are doing
  • Scheduling appointments
  • Assisting with referrals to other statewide programs such as WIC, FNS (Food and Nutrition Services), Medicaid, and Family Planning

Your care manager will talk with you on the phone, meet you in person at your prenatal visits, in your home or another location that is convenient for you

How to get connected?

Your provider will complete a pregnancy risk screening to identify any barriers or health concerns that may prevent you from having a healthy pregnancy.  Participants may also be referred by their prepaid Health Plan (PHP). Contracted PHPs include: WellCare, Healthy Blue, UnitedHealth Care, AmeriHealth Caritas and Carolina Complete Health. CMHRP accepts self-referrals and community referrals as well.
