Care Management for at Risk Children

CMARC works to improve the overall health of young children.  If your child qualifies for this program, your family can receive services offered by CMARC.

CMARC focuses on family strengths and concerns.  CMARC Care Managers are nurses and social workers who will help identify programs, services, and resources that meet your family’s needs through home visits, doctor visits, telephone calls, and other personal contacts.

Your Care Manager will serve as a link between you and your child’s doctor or nurse.  He or she will also help you link with other community resources and offer information and support.

Services Provided

  • Assessment or screening to identify needs and concerns
  • Assistance setting and meeting goals addressing prioritized needs and concerns
  • Parent/caregiver education related to needs or concerns
  • Support in identifying and addressing obstacles that prevent the family to reach goals
  • Education and strategies to reduce the impact of adverse childhood events
  • Linkage to a medical home
  • Linkage and referral of children and families to needed community programs and supports

Coordination of the child’s care to ensure appropriate medical care, social services and other supports are obtained to increase the quality of care and decrease the cost

How are children referred or identified?

  • Direct provider referrals
  • Social service agency referrals, such as Women, Infants and Children (WIC), DSS and community programs
  • Hospital referrals
  • Direct referral by enrollees or families

Risk stratification and other data-based identification methods by health plans

Location of Services

CMARC CMs meet with the family in the location of the family’s choosing, which could include:

  • home
  • child’s medical home
  • community location
  • videoconference
  • phone, if specific face-to-face contacts have occurred
