In-Home Aide & Home Delivered Meals

In Home Aide and Home Delivered Meals Services may help you remain safely in your home. If you are elderly and/or a disabled adult who is unable to prepare meals, and/or need assistance with personal care tasks, this may be the help you are seeking. Social Workers provide on-going case management services to ensure appropriate service provision and to assist the adult with aging safely in their home.

In-Home Aide Services

This program provides assistance with personal care. Assisted Daily Living (ADL’s) help with dressing, bathing, transferring, grooming and eating, and will also provide limited assistance with chores. Independent Assisted Daily Living (IADL’s) assist with cooking, laundry and light cleaning to enrich your overall health and well-being to prevent placement outside of the home. A functional needs assessment is required before services can be approved.

Home Delivered Meals

This program provides a hot nutritious daily meal to prevent malnutrition and hunger, and to enhance overall well-being.
