Assisting patients to navigate healthcare systems through education and connection to community resources

Assisting people who are experiencing unsheltered homelessness and other vulnerable populations
Assisting Familiar Faces
CPs achieved a 20-25% decrease in 9-1-1 calls for service by connecting patients to medications, primary care, arranging transportation to outpatient services, and providing preventive education
MAT Bridge Program
Durham County M.I.H. Community Paramedics are at the forefront of the opioid epidemic.
Durham CPs are now able to offer buprenorphine following an opioid overdose as well as connection to long term Medication Assisted Treatment (MAT).
