Permitted Industrial User Submittals

Review the below guidance for compliance submittals. Click the applicable submittal type for specific instructions. All files submitted MUST be named per the required naming convention. If the required naming convention is not followed appropriately and Durham County is unable to decipher the submittal, the files will not be reviewed for compliance. This may lead to a violation if not corrected within the reporting period timeframe.

Industry Compliance Monitoring & Flow Reports

Submittal Requirements:
 
1. Signed and Certified by the Authorized Representative of the User per section 26-132 (3) of the Durham County Sewer Use Ordinance.
 
2. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Month/Year of Report - Type of Document

Date of Submittal (YYMMDD) – This should be the date of the submittal. Use the specific format Year, Month, Day. Example: “210704” for submitting a report on July 4, 2021.

IUP Number – The Industrial User Permit Number will be in one of three formats: DC-###, DCL-###, or DCL-BSL###.

Industrial User Name – The name of the Industrial User or Permittee.

Month/Year of Report – This will refer to what month/period and year the samples or flow report represents.

Type of Document – flow report, sampling report, biosafety certification, Tenant IWS, Slug Spill Plan, NOV Response, Permit Violation Notification, Discharge Request, Request ATC, Permit Application, etc.

Ex:  220715 – DCL-001 – Carolina Carpets – June 2021 – Flow & Sampling

IUP Certifications (Biosafety Decon., Solids Interceptor, etc.)

Submittal Requirements:
 
1. Signed and Certified by the Authorized Representative of the User or tenant (if applicable). 
 
2. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Tenant (if applicable) - Type of Document

Date of Submittal (YYMMDD) – This should be the date of the submittal. Use the specific format Year, Month, Day. Example: “210704” for submitting a report on July 4, 2021.

IUP Number – The Industrial User Permit Number will be in one of three formats: DC-###, DCL-###, or DCL-BSL###.

Industrial User Name – The name of the Industrial User or Permittee.

Tenant (if applicable) – The name of the tenant for which the certification applies.

Type of Document – flow report, sampling report, biosafety certification, Tenant IWS, Slug Spill Plan, NOV Response, Permit Violation Notification, Discharge Request, Request ATC, Permit Application, etc.

Ex:  220715 – DCL-001 – Carolina Carpets – Durham Cell Science – Biosafety Cert

Notice of Violation (NOV) Response

Submittal Requirements:
 
1. Signed and Certified by the Authorized Representative of the User per section 26-132 (3) of the Durham County Sewer Use Ordinance.

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Type of Document

Permit Violation Notification

Submittal Requirements:
 
1. The “Industrial Self-Monitoring Permit Violation Notification” form shall be completely filled out and signed.
 
2. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Type of Document

Date of Submittal (YYMMDD) – This should be the date of the submittal. Use the specific format Year, Month, Day. Example: “210704” for submitting a report on July 4, 2021.

IUP Number – The Industrial User Permit Number will be in one of three formats: DC-###, DCL-###, or DCL-BSL###.

Industrial User Name – The name of the Industrial User or Permittee.

Type of Document – flow report, sampling report, biosafety certification, Tenant IWS, Slug Spill Plan, NOV Response, Permit Violation Notification, Discharge Request, Request ATC, Permit Application, etc.

Ex:  220715 – DCL-001 – Carolina Carpets – Permit Violation Notification

Slug Spill Plan

Submittal Requirements:
 
1. Slug Spill Plan shall meet the minimum requirements outlined in the Durham County guidance document titled “Guidelines for Slug Spill Plan”.
 
2. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Type of Document

Date of Submittal (YYMMDD) – This should be the date of the submittal. Use the specific format Year, Month, Day. Example: “210704” for submitting a report on July 4, 2021.

IUP Number – The Industrial User Permit Number will be in one of three formats: DC-###, DCL-###, or DCL-BSL###.

Industrial User Name – The name of the Industrial User or Permittee.

Type of Document – flow report, sampling report, biosafety certification, Tenant IWS, Slug Spill Plan, NOV Response, Permit Violation Notification, Discharge Request, Request ATC, Permit Application, etc.

Spill Report Form

Submittal Requirements:
 
1. The "Spill Report Form" shall be completely filled out and signed. 
 
2. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Type of Document

Date of Submittal (YYMMDD) – This should be the date of the submittal. Use the specific format Year, Month, Day. Example: “210704” for submitting a report on July 4, 2021.

Industrial User Name – The name of the Industrial User or Permittee.

Industrial Waste Survey (IWS) - Short Form (Tenants)

Submittal Requirements:
 
1. The "Industrial Waste Survey (IWS) Short Form" shall be completely filled out and signed per the "IWS Short Form Guidance" document.
 
2. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Tenant (if applicable) - Type of Document

Date of Submittal (YYMMDD) – This should be the date of the submittal. Use the specific format Year, Month, Day. Example: “210704” for submitting a report on July 4, 2021.

IUP Number – The Industrial User Permit Number will be in one of three formats: DC-###, DCL-###, or DCL-BSL###.

Industrial User Name – The name of the Industrial User or Permittee.

Tenant (if applicable) – The name of the tenant for which the certification applies.

Supplemental Data for Review (Biocide Discharge Request)

Submittal Requirements:
 
1. Complete the "Biocide Worksheet PT-101" or "Biocide Excel PT-101".
 
2. Provide the following required information:

  • A FULL DISCLOSURE safety data sheet (SDS) for biocide
  • Copy of source data for half-life
  • Copy of source data for inhibition criteria

3. Industry will be invoice per the current Durham County Utilities Fee Schedule at the time of submittal.

4. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Tenant (if applicable) - Type of Document

Date of Submittal (YYMMDD) – This should be the date of the submittal. Use the specific format Year, Month, Day. Example: “210704” for submitting a report on July 4, 2021.

IUP Number – The Industrial User Permit Number will be in one of three formats: DC-###, DCL-###, or DCL-BSL###.

Supplemental Data for Review (Other Discharge Requests)

Submittal Requirements:
 
1. Provide the following required information via the "Discharge Request Form":

  • A FULL DISCLOSURE safety data sheet (SDS)
  • Approximate volume of discharge 
  • Approximate concentration of chemical in discharge
  • Approximate frequency of discharge  (Ex: approximately 1 gallon of acetic acid at 1:50 dilution, once per week)

2. Industry will be invoice per the current Durham County Utilities Fee Schedule at the time of submittal.

3. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Tenant (if applicable) - Type of Document

Request Authorization to Construct (ATC)

Submittal Requirements:
 
1. A letter requesting ATC listing all proposed pretreatment units signed by the Authorized Representative of the User. Reference the "Authorization to Construct Guidance" document, if needed.
 
2. Plumbing plans for construction, stamped by a NC certified PE, with room details and highlighted process waste lines that show origination and use of all drain lines tied to pretreatment units. Durham County may require a meeting to discuss proposed changes.
 
3. All applicable specification sheets, schematics, details, etc.
 
4. Industry will be invoiced per the current Durham County Utilities Fee Schedule at the time of submittal.

5. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Tenant (if applicable) - Type of Document

Date of Submittal (YYMMDD) – This should be the date of the submittal. Use the specific format Year, Month, Day. Example: “210704” for submitting a report on July 4, 2021.

Industrial User Permit Application - Long Form (Permit Modification or Renewal)

 Submittal Requirements:
 
1. Answer all questions on the "IWS & Permit Application - Long Form" and have it signed by the Authorized Representative of the User per section 26-132 (3) of the Durham County Sewer Use Ordinance. 
 
2. Include all requested attachments such as site plan, flow schematics, etc.
 
3. Industry will be invoiced per the current Durham County Utilities Fee Schedule at the time of submittal.

4. File(s) shall be named per required naming convention:

Date of Submittal (YYMMDD) - IUP Number - Industrial User Name - Tenant (if applicable) - Type of Document
