How Can I Obtain My Patient Record or Bill?
All persons assessed, treated or transported by Durham County EMS have a Patient Care Report (PCR) that is stored securely. Patients who are treated and/or transported may also receive a bill of service.
In order to ensure compliance with federal law (see Patient Privacy and HIPAA), we require all patients requesting records or bills to fill out the Durham County EMS Authorization to Use and Disclose Protected Health Information:
To obtain a copy of a Patient Care Report (PCR) or bill, the patient must fill out the above form and email it along with a copy of their photo ID to emsrecords@dconc.gov. Alternatively, they may bring a copy of their ID and fill out the form in person at the OES Administration office, Monday-Friday, 8AM-5PM:

Under most circumstances, patient records and bills cannot be released to spouses or anyone except the patient themselves without the patient’s signature. To obtain patient records for a child, a dependent adult, or someone who is deceased, your own photo ID must be presented along with documentation of your relation to the patient (parent, legal guardian, medical power of attorney, next of kin, etc.) Please see the Authorization Verification Instructions linked above for more details.
Be advised that an EMS Patient Care Report (PCR) is not a substitute for a Physician’s (Doctor’s) note of illness.
